Documenting is telling the story of how we are caring for the resident, how they evolve and what we are doing to adapt in meeting their needs. It is critical for determining if the appropriate standard of care is being provided to the resident. Failure to chart and poor communication is hard to defend. The concept of person-centered care requires that we plan, document and accompany the person in the most individualized way so that they have the best possible outcomes based on their current needs.
